For a non-emergency medical transportation company, the vehicle is the business. If a van is down, late, or involved in an accident, the impact is immediate. Commercial auto insurance is one of the core policies underwriters review when they consider a NEMT account.

NEMT fleets can be hard to place because they combine passenger transportation, medical-related trips, scheduled routes, hired drivers, and often higher vehicle values. Carrier appetite varies by state, radius, driver history, vehicle type, years in business, and loss history.

What underwriters usually want to understand

  • How many vehicles are in the fleet
  • Whether vehicles are wheelchair accessible
  • Vehicle year, make, model, VIN, seating capacity, and garaging address
  • Driver motor vehicle records and hiring standards
  • Trip types, including medical appointments, facility contracts, private pay, or brokered transportation
  • Operating radius and states of operation
  • Prior insurance, current limits, and loss runs
  • Safety procedures, vehicle maintenance, and incident reporting

California operators may also need to think about contracts, facility requirements, and state or local transportation rules. Operators in other states face similar issues, but the exact requirements can vary. Insurance is only one part of compliance, so it is important to review contract and regulatory obligations with the right professional.

Common commercial auto coverage areas

A NEMT auto policy may include liability, physical damage, uninsured or underinsured motorist coverage, medical payments or personal injury protection where applicable, and hired and non-owned auto depending on the account and carrier. Available options depend on underwriting, carrier appetite, policy terms, conditions, and exclusions.

Do not assume a personal auto policy or a standard business auto policy is built for passenger transportation. NEMT work should be disclosed clearly, including wheelchair transport, attendants, contracted trips, and any paid passenger services.

How to prepare before requesting terms

  • Keep current driver lists and MVRs organized
  • Document vehicle maintenance and inspection procedures
  • Collect at least three to five years of loss runs if available
  • Be clear about contracts, trip volume, and operating radius
  • Separate NEMT operations from any unrelated business activity
